Telehealth and Patient Engagement: With telehealth technologies, patients are taking more control of their well-being. Home Health Care And Remote Monitoring Systems: Care that is provided to patients and their families in their residences to promote, maintain or restore health; or to minimize the effects of illness and disability, including terminal illnesses. In Medicare enrollment data and claims as well a Medicare Current Beneficiary Survey, home health care is defined as home visits made by professionals, including physicians, nurses, home health aides, therapist and social workers. The use of interactive devices and remote monitoring enable a patient’s vital signs to be sent on a regular basis to health care providers without the patient having to travel.
The doctors can treat flu symptoms, rashes, allergies, urinary tract infections, and bronchitis—and even prescribe medication—without ever physically seeing the patient. Telemedicine for intensive care unit (ICU) rounds: Telemedicine is also being used in some trauma ICUs to reduce the spread of infections. Rounds are usually conducted at hospitals across the country by a team of approximately ten or more people to include attending physicians, fellows, residents and other clinicians. This group usually moves from bed to bed in a unit discussing each patient. This aids in the transition of care for patients from the night shift to the morning shift, but also serves as an educational experience for new residents to the team. A new approach features the team conducting rounds from a conference room using a video-conferencing system. The trauma attending, residents, fellows, nurses, nurse practitioners, and pharmacists are able to watch a live video stream from the patient's bedside. They can see the vital signs on the monitor, view the settings on the respiratory ventilator, and/or view the patient's wounds. Video-conferencing allows the remote viewers two-way communication with clinicians at the bedside.[42]
Telehealth allows the patient to be monitored between physician office visits which can improve patient health. Telehealth also allows patients to access expertise which is not available in their local area. This remote patient monitoring ability enables patients to stay at home longer and helps avoid unnecessary hospital time. In the long-term, this could potentially result in less burdening of the healthcare system and consumption of resources.[1][8]

As the potential -- if not the actual success, yet -- of AI grows in healthcare, telemedicine stands to benefit from it. It's not hard to imagine telemedicine chatbots being the initial party that a patient discusses symptoms with during a smartphone video call, and based on AI deductions of the situation, recommendations could follow or an actual physician could join the discussion.
While the industry is still a long way from a standard set of established guidelines for telemedicine, the American Telemedicine Association has put together guidelines for a range of specialties based on a survey hundreds of research study. What are the clinical, technical, and administrative guidelines a medical practice needs to put in place when they’re adopting telemedicine? Beyond the minimal legal requirements of that state, what are telemedicine best practices?
An example of these limitations include the current American reimbursement infrastructure, where Medicare will reimburse for telehealth services only when a patient is living in an area where specialists are in shortage, or in particular rural counties. The area is defined by whether it is a medical facility as opposed to a patient's' home. The site that the practitioner is in, however, is unrestricted. Medicare will only reimburse live video (synchronous) type services, not store-and-forward, mhealth or remote patient monitoring (if it does not involve live-video). Some insurers currently will reimburse telehealth, but not all yet. So providers and patients must go to the extra effort of finding the correct insurers before continuing. Again in America, states generally tend to require that clinicians are licensed to practice in the surgery' state, therefore they can only provide their service if licensed in an area that they do not live in themselves.[1]

Primary-care clinicians learn from their specialist mentors at the university hubs, but they also learn from each other. And by applying the knowledge they gain in the field, they produce new knowledge, which they then relay to their specialist and primary-care colleagues in their learning community.


As the CCHP notes, different organizations have different definitions for telehealth. California very specifically defines it as “the mode of delivering healthcare services and public health via information and communication technologies to facilitate the diagnosis, consultation, treatment, education, care management and self-management of a patient's healthcare while the patient is at the originating site and the healthcare provider is at a distant site. Telehealth facilitates patient self-management and caregiver support for patients and includes synchronous interactions and asynchronous store and forward transfers.” The Health Resources and Services Administration (HRSA), meanwhile, defines it as “the use of electronic information and telecommunications technologies to support long-distance clinical healthcare, patient and professional health-related education, public health and health administration.”


According to this 2015 Cardiac Implantable Electronic Device (CIED) study, patients whose implantation included remote monitoring capabilities had a higher rate of survival than patients without it. “ Furthermore, according to the Center for Technology and Aging, patients who participated in RPM were less likely to experience hospital stays, incurred fewer ED and urgent-care visits, and reported better management of their symptoms. They also indicated increased physical stamina as well as greater overall patient satisfaction and emotional well-being.
