34 states and the District of Columbia require that private insurers cover telehealth the same as they cover in-person services. Many other insurers cover at least some telehealth service--and many more have expressed interest in expanding their telehealth coverage. Policies and regulations in the telemedicine arena can be confusing for providers, vendors, and payers. Organizations interested in implementing telemedicine should be familiar with the laws in their state. For example, some states require informed consent from patients, while others do not. Some payers may not pay the same rate for telemedicine services as they do for in-person services. Practices should identify how providers will be paid, as some organizations seek grant funding.


Telehealth, the use of electronic communication to remotely provide health care information and services, is gaining more and more attention as providers, patients, and payers all seek more effective and cost-efficient ways to deliver care. Physical therapy is no exception, and while those services have developed mostly in rural areas to accommodate the long distances between patients and providers, telehealth in physical therapy is being considered in other geographic and clinical settings.
Real-time telemedicine (also called “synchronous telemedicine”) is probably what most people first think of when they hear “telemedicine.” Real-time telemedicine requires a live interaction between either a health professional and patient, or between health professionals, using audio and video communication. Think videochat. While most real-time telemedicine software is much more sophisticated than a simple videochat platform, the basic goal is to both see and talk to the patient from afar. This type of telemedicine is meant to offer a virtual alternative to the in-person doctor’s visit.
Telemedicine companies that are consumer-facing offer the huge benefit of on-demand care for patients. A sick patient can simply login online and request a visit with one of the company’s doctors and get treatment. But this model, similar to the retail health movement, leads to a breakdown in care continuity. A random doctor who doesn’t know the patient, doesn’t know their whole medical history. The best approach to telemedicine? Providing tools to providers to easily connect with their own patients. 
According to this 2015 Cardiac Implantable Electronic Device (CIED) study, patients whose implantation included remote monitoring capabilities had a higher rate of survival than patients without it. “ Furthermore, according to the Center for Technology and Aging, patients who participated in RPM were less likely to experience hospital stays, incurred fewer ED and urgent-care visits, and reported better management of their symptoms. They also indicated increased physical stamina as well as greater overall patient satisfaction and emotional well-being.
According to an SEC filing relating to its recent fundraising round, Teladoc brings in between $25 million and $100 million in sales. The company is also experimenting with taking a cut of the cost savings it delivers to health plans, which could boost revenue further. Revenue grew 75% in 2012, 100% in 2013 and is expected to grow another 100% this year, Gorevic says.
In August 2017, hospitals across the country were penalized with Medicare reimbursement cuts due to high 30-day readmission rates. The potential to reduce these rates and avoid penalties has made telehealth a financial priority. Healthcare provider TripleCare was the subject of a study conducted by the TRECS Institute, which found that virtual physician services had both increased care quality and averted 91 unnecessary admissions. The result was $1.3 million in Medicare savings. Telehealth services can also provide patients and healthcare facilities in rural areas with additional benefits. Due to its digital nature it is often assumed that telehealth saves the health system money. However, the evidence to support this is varied. When conducting economic evaluations of telehealth services, the individuals evaulating them need to be aware of potential outcomes and extraclinical benefits of the telehealth service.[37] Economic viability relies on the funding model within the country being examined (public vs private), the consumers willingness-to-pay, and the expected remuneration by the clinicians or commercial entities providing the services (examples of research on these topics from teledermoscopy in Australia [38][39][40]).

Advance Medical is a telemedicine company which has locations in Chile, Spain, and parts of Asia. It runs a virtual doctor service, called Global Care on Demand, which offers access to medical advice by phone or video by doctors located in eight main hubs around the world who speak more than 20 languages, and is targeted at expatriates.[30]

Billions of investment dollars have been poured into apps and websites that offer this virtual consultations with physicians, ranging from Doctor on Demand to American Well. The theory behind them is that millennials would opt for a digital alternative to an in-person physician's visit, if the option were available. And patients in remote, rural areas who are miles away from the nearest doctor would have few alternatives.
