“Telemedicine is the natural evolution of healthcare in the digital world,” American Telemedicine Association. Telemedicine empowers the caregivers to remotely interact with their patients, which greatly improves both the efficiency and affordability of healthcare. Today patients, doctors and caregivers have learned to accept telemedicine (often called ‘telehealth’ or ‘connected health’) as one of many ways of delivering care.
“Another distinction between telemedicine and D2C telehealth is that telemedicine consultations are often with medical specialists like cardiologists, dermatologists and pulmonologists,” Downey continued. “These often occur when the patient is in an underserved rural community and the specialist is in a large urban area. The distance makes it difficult to make and keep appointments otherwise. D2C telehealth, on the other hand, best deals with minor primary care issues over the phone. If deemed to be a more serious health concern, the patient is told to make an appointment with a specialist or to proceed to a hospital emergency room.”
Telehealth involves the distribution of health-related services and information via electronic information and telecommunication technologies. It allows long distance patient/clinician contact and care, advice, reminders, education, intervention, monitoring and remote admissions. As well as provider distance-learning; meetings, supervision, and presentations between practitioners; online information and health data management and healthcare system integration. Telehealth could include two clinicians discussing a case over video conference; a robotic surgery occurring through remote access; physical therapy done via digital monitoring instruments, live feed and application combinations; tests being forwarded between facilities for interpretation by a higher specialist; home monitoring through continuous sending of patient health data; client to practitioner online conference; or even videophone interpretation during a consult.

Disease Management: A coordinated and continuous health process for the purposes of managing and improving the health status of a specifically defined population of patients over the complete course of the disease (e.g., DM, CHF). The targeted patient populations are high-cost, high-risk patients that have chronic conditions that require appropriate care in order to be maintained properly.
Bluetooth Wireless: Bluetooth refers to an industrial specification that applies to wireless area networks. Bluetooth technology offers a way of connecting and exchanging information between devices, including laptops, mobile phones, PCs, video game consoles, digital cameras and printers over a globally unlicensed and secure short-range radio frequency. The Bluetooth Special Interest Groups has developed and licensed the Bluetooth specifications.
Asynchronous or "Store and Forward": Transfer of data from one site to another through the use of a camera or similar device that records (stores) an image that is sent (forwarded) via telecommunication to another site for consultation. Asynchronous or "store and forward" applications would not be considered telemedicine but may be utilized to deliver services.
The rise of the internet age brought with it profound changes for the practice of telemedicine. The proliferation of smart devices, capable of high-quality video transmission, opened up the possibility of delivering remote healthcare to patients in their homes, workplaces or assisted living facilities as an alternative to in-person visits for both primary and specialty care.
But investors had other worries that weren't related to the broader market. Teladoc Health CFO and COO Mark Hirschhorn sold more than $700,000 of his stock right after the company's third-quarter earnings update. A short-seller posted an online article predicting that sales for one of Teladoc's fastest-growing businesses would soon fall. These two stories hit on the same day. Teleradiology : This is the transfer of radiological images. X-Rays, MRIs and CTs are all types of radiological images. These images are used for consultation, diagnosis or interpretation. They can be transferred through satellite connections, local area networks or even standard telephone lines. The Picture Archiving and Communications Systems allow centralized storage and the access of these images over information systems such as computers.
Medicaid guidelines require all providers to practice within the scope of their State Practice Act. Some states have enacted legislation that requires providers using telemedicine technology across state lines to have a valid state license in the state where the patient is located. Any such requirements or restrictions placed by the state are binding under current Medicaid rules.

The Health Resources Services Administration defines telehealth as the use of electronic information and telecommunications technologies to support long-distance clinical health care, patient and professional health-related education, public health and health administration. Technologies include videoconferencing, the internet, store-and-forward imaging, streaming media, and terrestrial and wireless communications.
Today, 95 percent of Americans own cell phones and 77 percent own smartphones. These and other mobile devices can be leveraged to promote better health outcomes and increased access to care. mHealth or mobile health refers to healthcare applications and programs patients use on their smartphones, tablets, or laptops. These applications allow patients to track health measurements, set medication and appointment reminders, and share information with clinicians. Users can access hundreds of mHealth applications including asthma and diabetes management tools as well as weight loss or smoking cessation apps. Additionally, mobile devices allow users to schedule appointments and communicate with providers via video conference and text message.
Internet Protocol (IP): The protocol for sending data from one computer over the Internet to another. Every computer that is on the Internet has one address at least that identifies it uniquely from all of the other computers that are on the Internet. Internet Protocol is a connectionless form of protocol, meaning there isn’t a connection that is established between the two points that are communicating with one another. A videoconferencing system’s IP address is its telephone number.
There are a variety of payment models to fund telemedicine services. For example, some health systems offer telemedicine consultations as part of their regular care services, and payers charge patients based on insurance plans or government reimbursement schedules. In other cases, a patient's employer offers virtual care options as part of health insurance coverage premiums. Some people may opt to independently use a telemedicine vendor for a flat fee.

RPM telemedicine is quickly rising in popularity as more health professionals realize its potential effects on chronic care management. For instance, a patient with diabetes who has a glucose tracker in their home can measure their glucose levels at regular intervals and transmit them to their doctor. If all is well, those results are simply recorded. If something looks off, the physician may flag it and call in the patient for a consult.

However, since providers are licensed to practice in a specific state, they are only legally allowed to offer telemedicine services to patients in the same state. NIDRR's grantees include the "Rehabilitation Engineering and Research Center" (RERC) at the University of Pittsburgh, the Rehabilitation Institute of Chicago, the State University of New York at Buffalo, and the National Rehabilitation Hospital in Washington DC.
